A Ukrainian residence permit file has four core items: passport with the Type D visa and copies, a certified Ukrainian translation of its data pages, medical insurance and the fee slip. Then the ground: for work, the work permit and the employer's undertaking to notify DMSU if the contract ends. File in person, 15 working days before the stay ends.
Two of the refusal grounds DMSU lists are an incomplete package and a late filing. The State Migration Service publishes the list by ground. This page sets it out for four grounds: work, study, founder and family, with the fees, the deadline and the Pakistani documents that need an apostille first.

For a Pakistani worker the whole file is short: the core four plus the work permit and the employer's undertaking. The permit then runs for the term of the employment contract. The founder ground applies only from EUR 100,000 in the Ukrainian company's capital (Law No. 3773-VI, Art. 4) and gives a two-year permit; what the company itself must show is in company documents. Students get a permit for the study period in the university's order.
Pakistani documents in the residence file
On the work, study and founder grounds the Pakistani paperwork is your passport and its translation. Family grounds add Pakistani civil records: a Nikah Nama for a spouse, a birth certificate for a child. These take the MoFA Pakistan apostille first, then a Ukrainian translation of the document together with its apostille, with the translator's signature certified by a notary (Law “On Notariat”, Art. 79). The lists in Resolution No. 322, paras. 32–33, carry no police certificate, and proof of funds appears only where a family member joins a foreign permit holder. Documents outside the list cost fees and weeks; bring what your ground asks for.
Fees, deadline and how long it takes
The DMSU service page lists a government charge of UAH 34, an administrative service fee of UAH 452 and UAH 654 for the blank card. File no later than 15 working days before your permitted stay ends (Resolution No. 322, para. 17), in person, at a DMSU territorial office, a DMSU state enterprise or an administrative service centre for your place of residence. The permit is issued within 15 working days (para. 5). An incomplete package or a late filing is a ground for refusal, and so is a stay already over its term. When the permit expires it is exchanged: the next file can go in up to its last day, and with fewer than 15 working days left only at a DMSU territorial office (para. 19).
