The Embassy of Ukraine in Pakistan is at House 6, Street 29, sector F-7/1, Islamabad: Monday to Friday, 9:00–17:00, +92-51-260-89-66, [email protected]. Visa files go through VFS Global: register the form on visa.mfa.gov.ua, lodge it at the Islamabad, Karachi or Lahore centre, and the consul decides. Work visa D-04: USD 404.
The embassy decides, and VFS Global handles the paperwork around the decision. The embassy's own visa page says so, and it sets the order: online form, VFS appointment, documents at the centre, decision by a consular officer, passport back at the same centre. The visa centres are run by VFS Global under contract with the embassy. They take in applications, help fill in and register forms and sell optional services such as photos, copies, translation, insurance options and courier delivery; the decision on every application is made by Ukraine's consular officers only (embassy visa page). Embassy tariff “Visa Fees (USD, from August 01, 2024)”, row for Pakistan. The VFS service charge is paid on top, in rupees, and the consular fee at designated Faysal Bank branches. What the embassy asks for
Every visa file needs a passport valid at least three months after the planned departure from Ukraine, issued within the last 10 years and with two blank pages; the registered and signed form; one colour photo 35 × 45 mm; health insurance of at least EUR 30,000; proof of funds, which the embassy states as USD 750 per 15 days of stay; and the consular fee. The Type D visa adds the document behind your ground: for work, a notarised copy of the work permit; for study, an invitation from a Ukrainian university registered under Ministry of Education rules; for family, the marriage or family documents. Pakistani documents are apostilled by MoFA and translated into Ukrainian; the embassy accepts an English translation for family documents when a Ukrainian one is not possible.
A Type D visa is issued only by a Ukrainian mission abroad, as a multi-entry visa for 90 days; inside Ukraine you then apply for a residence permit. If the embassy refuses the visa
The refusal is stamped in the passport, dated and signed, and you are told the grounds. The embassy lists ten, among them a forged or damaged passport, knowingly false information, missing insurance, unproven funds and an unproven purpose of stay. You can apply again, or send the embassy a letter of appeal with supporting evidence within 60 working days of receiving the refusal (Resolution No. 118, para. 30). The consular fee is not refunded, so the stated ground is what to fix first; a lawyer can read it with you in a legal consultation.

Every step above has an official channel: the MFA form, the VFS booking site, the fee at the centre, the consul's decision.
